This assessment provides an opportunity for you to evaluate an existing care coordination program as the initial step in instituting practice changes that can positively influence health and wellness outcomes.

    ASSESSMENT INSTRUCTIONSComplete the assessments in this course in the order in which they are presented.PREPARATIONFor this assessment, you will assume the role of an external health care efficiency consultant to your care coordination unit or organization. Leaders in your organization are investigating potential changes to the provision of coordinated care that would improve overall quality, safety, and cost effectiveness. Because of your proven leadership and expertise in care coordination, you have been asked to evaluate the existing care coordination program and submit your findings to the administration in an evaluation report.Note: As an alternative to using your own unit or organization as the practice setting for this assessment, you may use the Hypothetical Practice Setting [PDF] linked in Capella Resources, as the basis for your program evaluation.Note: Remember that you can submit all, or a portion, of your draft evaluation report to Smarthinking for feedback before you submit the final version for this assessment.       • Describe the level of support provided by current practices for the organization’s mission, vision, and values.
      • Consider the policy and regulatory guidelines and standards that should determine current practice.
    • Explain how interprofessional collaboration among various disciplines can facilitate the achievement of health goals for patients, the community, or a population.
      • What evidence do you have to support your assertions and conclusions?
    • Write clearly and concisely, using correct grammar and mechanics.
      • Express your main points and conclusions coherently.
      • Proofread your writing to minimize errors that could distract readers and make it more difficult for them to focus on the substance of your evaluation.
    • Support main points, claims, and conclusions with relevant and credible evidence, correctly formatting citations and references using current APA style.
      • Is your supporting evidence clear and explicit?
      • How or why does particular evidence support a claim?
      • Will your audience see the connection?
